You will receive recommendations on diet, nutrients, exercise and lab testing if needed, in a step-by-step plan for success!

It’s important that you take your time and are as detailed as possible when recording your information. Tell me specifically what you are looking for. You may include any extra documentation, tests or summaries that you wish. The information that you provide is what is used to create your program, the quality or your information will determine how your program is written. Please make copies of your forms before you mail them. Your forms will not be returned to you.

2. Fill out all health questionnaires completely.

4. 30 Day rotational meal plan is optional

Your custom health program will be mailed to you with in 14 days of receipts of forms. You will receive a nutrient protocol, dietary plan and exercise program. All nutrients suggested are available through my office or you may use your preferred vendor.
